The Receipts Co. provides tools to help you organize your own observations and appointment notes.

What these tools do

Our tools do not diagnose conditions, recommend treatment, or replace a qualified professional’s advice. Using a worksheet does not create a clinician-patient or attorney-client relationship. We cannot promise a diagnosis or a particular response from a provider.

Personal use

You may print and reuse the free Appointment Receipt for yourself and share your completed record with your care team. Please link to the original page when sharing the blank tool. Do not sell or redistribute our products as your own.

Purchases and outside links

Purchases take place on Etsy. Check the listing for product details and delivery information. Etsy’s policies cover payment and applicable refund terms. Etsy and Pinterest are separate services with their own terms.

Availability and questions

We may update the site and tools. We aim to keep them accurate and available, but cannot guarantee that they will be error-free or always accessible. Nothing here limits rights that cannot be excluded under applicable law.
