Your symptoms.
On the record.

For people whose symptoms have been dismissed, minimized, or missed.

Documentation kits that turn what you notice into a written record you can bring into the room, even when the words do not come easily. The Pivot Kit is the first rebuilt as a fillable PDF.

A short visit is a hard place to remember everything.

Many people say they have left an appointment feeling unheard, without the details written down and without a clear next step. Pain, fatigue and stress make it harder to recall dates and wording on the spot.

A written record does not depend on memory.

  • When it started and how it changedThe order of events, with dates.
  • What it did to your daySleep, work, mood, movement, in your own words.
  • What was said and decidedWho recommended what, and what was declined.
  • The questions you meant to askWritten before you walk in.

How it works

Three short steps that start weeks ahead and end when you leave the room. Each uses a page you can print.

  1. Weekly grid page from The Pivot Kit with boxes for daily counts and ratings.

    Track the pattern

Write down what happened, when, and how it affected your day. A weekly log shows patterns that single bad days hide.

  2. Provider scripts page from The Pivot Kit with three lines to read at an appointment.

    Bring the record

Take your log and the lines you want to say. Scripts give you wording for the moments when it is hard to find your own.

  3. The Receipts Co.

    Appointment Receipt

    Keep the receipt

After the visit, write down what you reported, what they said and what happens next. Keep the page with your records.

The kits

One kit per conversation. The Pivot Kit is the first rebuilt as a fillable PDF. The others are being rebuilt the same way, and each card will link out when its kit is ready.

Perimenopause

The Pivot Kit

For the changes you keep trying to explain. Six tools across 10 pages. A twelve-month timeline and a cycle log track the pattern. Daily trackers log hot flashes and night sweats, then sleep and brain fog. A decision memo and a one-page summary get you ready for the visit.

Which kit is for you

Start with what you want to bring to the appointment. The kits help you organize what you notice. They do not diagnose.

Fill it on a screen, or print it

The Pivot Kit is a fillable PDF: you type into 532 fields. A print-only version with no form fields is included for handwriting, so you can use it without any app.

Where the fields fill

  • Adobe Acrobat Reader (what the file is designed for)
  • Chrome's built-in PDF viewer (verified)

These do not fill the fields

  • GoodNotes
  • Notability

We have not tested other apps, phones or tablets, so we do not promise them. If you are unsure, use the print-only version.

Why we built this

Appointments are short, and the details that matter are easy to lose: the date a symptom began, how it changed, what was said, what was declined. The Receipts Co. makes printable documentation kits, and the Pivot Kit is also fillable, so those details end up on paper.

We are a small independent shop. We are not a clinic and we do not diagnose or recommend treatment. The kits are educational organization tools, written in plain language and built to help you prepare for a conversation with your own provider.

Questions

Which apps fill in the form fields?

The Pivot Kit is designed for Adobe Acrobat Reader, and the fields are verified to fill in Chrome's built-in PDF viewer. Open the PDF in one of those, click a box, and type. We have not tested other apps, phones or tablets.

Does it work in GoodNotes or Notability?

Those two apps do not fill PDF form fields. Use the print-only version, which has no form fields, to handwrite on paper.

Are all the kits fillable?

Not yet. The Pivot Kit is the first rebuilt as a fillable PDF. The other kits are being rebuilt the same way, and this site will say so when each one is ready.

Can I print it?

Yes. The Pivot Kit comes in US Letter and A4, and the download includes print-only files with no form fields. Print the whole kit or only the pages you need, and fill them in by hand.

Will a record get my symptoms taken seriously?

A record gives you dates, patterns and exact wording to discuss, and it can make a short visit more focused. It cannot promise how a provider will respond.

Is this medical advice?

No. These are educational organization tools. They do not diagnose conditions or recommend treatment, and they are not a substitute for a clinician.

Where do I buy the kits?

The Pivot Kit is on Etsy as an instant digital download. The listing shows what is included and the current price.

Does the free receipt collect my notes?

No. The Appointment Receipt is a printable page you fill in on paper, and nothing you write is sent to us. The email form only asks for your address.
